Patient Portal for Silicon Valley Sports Medicine
Our EMR system is epic and hosted by John Muir Health.  This is our patient records, schedule and also our Patient Portal.

The portal is called "MyChart" and is provided to our patients by John Muir Health. If you have any trouble logging in, please call the Patient Support Center @ (925) 941-2001. MyChart is fully HIPPA compliant in order to protect the privacy and security of health information beyond a standard website. 

The Patient Portal "MyChart", will give you access to:

• Appointments: Keep track of appointments
• Health History: Input or update your pertinent medical history
• Upload documents: Via "send message" attachment
• Lab Results: Request and view lab results
• Medical Records: View personal health record
• Messages: Send & receive messages from our staff
• Reminders: Receive health reminders
• Demographic Information: Update demographic information
• Referrals: View and request referrals

You will receive a welcome email from the email that will include a link to activate your MyChart account. If you need this message resent, just send us a quick text to (408) 376-3350.

What is MyChart?

MyChart is a website & app (ios & Android) that helps you organize your health information. It is a replacement portal for Patient Portal/Healow

 

How much does it cost?

As before, this is a free service setup and provided by our Electronic Medical Record (EMR) provider.

 

How can I sign up?

Just call or text the office @ 408-376-3350 and provide us with an updated email address. You will then recieve an email with an activation link for your MyChart account.

I haven't received my activation email - what do I do?

Please check your spam folder for an email from the domain @email.johnmuirhealth.com. If you still don't see it, please call or text the office @ 408-376-3350

 

Who is Epic?

Epic is the software company that provides the software for John Muir Health to provide our new EMR and new patient portal "MyChart".
