Sending (uploading) Documents and Images to MyChart

Sending documents & images (like Patient Registration or insurance card images) is an easy & secure process: you will compose a New Medical Question and attach the files.  These files go directly into your chart and are available to staff and providers as needed. 

If you have documents to send, save them as PDF (or Print / Microsoft Print to PDF).  You can also attach images (BMP, GIF, JPEG, JPG, PNG, TIF, TIFF)

MyChart Setup

Make sure you've set up your MyChart patient portal account by clicking on the activation email sent to you. Our new patient portal is MyChart by John Muir Health.  We have sent you an activation email which comes from donotreply@johnmuirhealth.com and has the subject "Signup information for MyChart". 

Please let us know if you need us to resend an activation or if you'd like it texted to you.  If you've previously signed up and forget your password, password reset can be done easily on the MyChart site.

Login to MyChart

In order to login, go to https://www.johnmuirhealth.com and click on MyChart Login upper right:

This will bring you to the Login page:




Once logged in, click on envelope icon next to provider's name "Send a message to TIFFANY GORMAN, MD" (you can choose any provider):
 

Fill in form: Choose any provider; Choose Non-Urgent Medical Question; Type a quick note & attach documents by clicking on "Attach an image" or "Attach another image" and then click "Send":



You will then see confirmation message (with attachments) has been sent.
